Register for DICOM Training/Workshop in Denmark 2020

Initial information needed

If you (and your colleagues) want to register for this 2-day DICOM Training/Workshop, please supply me the following information:

List with name(s) and e-mail address(es) of participant(s);
Which person(s) to send the invoice(s) to, also including their e-mail address(es);
If a company or hospital is going to pay the invoice(s), please also supply the official company/hospital name, address and VAT number;
Preference of payment: wire transfer or credit card.

The fee for this 2-day DICOM Training/Workshop, for each person participating, is 10,200 DKK excl. VAT;
Early bird discount of 10% when registering before March 28th 2020;
When 3 or more people from the same company/hospital participate, an additional discount may be applicable, please e-mail me for more information;
When the preferred DICOM Training/Workshop has not been fully booked yet, participants will be put on the reservation list and the invoice(s) will be sent.
To avoid unnecessary payments, invoices will only be sent after a minimum total number of participants is available for a specific Training/Workshop;
Reservations will remain valid for exactly one month, starting from the invoice date. When fully paid within one month, reservations become final.

If a participant cancels the Workshop more than one month before the start of the Workshop, a 75% refund will be offered. No refund will be offered after this period.
When a participant is unable to attend and would like another person to take his/her place, a 5% administration fee will apply.
In the unlikely event that the Workshop will be cancelled:
- When mutual agreed, another 2-day DICOM Training/Workshop may be participated in;
- Else, a full refund will be offered.
